The DIR Hangover One Year Later: How Have Pharmacists Fared?

This issue’s cover story took a look back at how pharmacies were impacted by a 2022 Centers for Medicare & Medicaid Services (CMS) ruling. “In 2022, [CMS] issued a ruling that eliminated retroactive DIR fees. Prior to the ruling, various retroactive fees would often be imposed on pharmacies weeks or months after prescriptions had been filled,” wrote Lois Levine, MA. Because of the ruling, pharmacies were forced to handle DIR fees from before 2024 while also paying consequential DIR fees at the point of sale going forward, something independent pharmacies were not used to. Referred to as a “double-whammy” for smaller chains, Levine dove into the phenomenon in this month’s issue.
